What is happening? Some substances like cannabis, MDMA (ecstasy), and psilocybin (magic mushrooms) affect brain chemistry, especially serotonin and perception pathways. While they may be used recreationally, they can also trigger intense anxiety, panic attacks, and feelings of depersonalization (feeling disconnected from yourself or reality).
Why does this occur?
These substances can overstimulate the brain’s emotional and sensory systems
They may alter perception, leading to fear, loss of control, or “unreal” sensations
Mixing substances (or combining with psychiatric medications) increases risk
Individual vulnerability (anxiety, trauma history, genetics) plays a major role
Common symptoms
Sudden intense fear or panic
Rapid heart rate, shortness of breath
Dizziness or feeling faint
Feeling detached from your body (depersonalization)
Feeling like the world is unreal (derealization)
Fear of dying or “going crazy”
These symptoms can occur during use or even after the substance wears off, and in some cases may persist.
Important clinical considerations
Psychedelics and MDMA commonly cause acute anxiety and psychological distress, especially at higher doses or in uncontrolled settings
Cannabis and hallucinogens are known triggers for panic and depersonalization episodes
Combining serotonergic substances with medications (e.g., antidepressants, MAOIs) can increase risk of adverse reactions
Co-use of multiple substances significantly increases unpredictability and harm
